Becoming a Licensed Professional Counselor (LPC) in Texas requires 3,000 hours of direct and indirect counseling experience and supervision. With Track Your Hours, you can populate the forms required by the Texas Behavioral Health Executive Council (BHEC) with the hours you log in your account.
You can generate and download these forms from your account, then sign and submit them outside of Track Your Hours.

Understanding the forms in your account
There are 3 forms that you can generate from your Track Your Hours account:
Supervised Experience Documentation Form
The Supervised Experience Documentation Form is provided by the BHEC. You must complete this form for each supervisor and submit it with your application for licensure.
This form summarizes your supervisory relationship(s) and outlines the experience you’ve gained under each supervisor. It consists of 2 pages. The first page is completed by you:

While Track Your Hours will populate certain fields related to your site, supervisor, and hours, you and your supervisor will need to complete the rest of the form outside of Track Your Hours.
Note: If you download the Supervised Experience Documentation Form for multiple supervisors at once, a separate 2-page form will be generated for each supervisor.
Supervision Log
The Supervision Log is provided by the BHEC. This can be used to track your weekly supervised experience hours. While this isn’t required to be submitted with your license application, your supervisor may ask you to complete this form so they can review your hours.
Track Your Hours will populate your weekly hours, with up to 12 weeks included on each page.
The Supervision Log includes columns for Direct Supervision Hours, Direct Clinical Hours, Indirect Hours, and Total Experience Hours Earned. The Total Experience Hours Earned column sums your Direct Clinical Hours and Indirect Hours for each week. Your Direct Supervision Hours aren’t included in this column because they’re considered separate from your experience hours.

You and your supervisor will need to complete the Supervision Session Content, LPC-S Initials, and LPC-Intern Initials columns outside of Track Your Hours.
Notes
The Notes report is created by Track Your Hours and can be used to easily reference any weekly notes you entered while logging hours. You shouldn’t submit this report with your license application.
When generated, the Notes report will populate all notes within a date range, organized by week, then by site and supervisor.
Generating and downloading forms
To generate and download a form:
- Navigate to the Forms page
- Choose the Form or report you'd like to create
- Select a Supervisor and site
- Choose All hours or specify a Date range
- Click Download PDF
Note: If you have multiple supervisors, you can choose to create a form for one supervisor at a time, multiple supervisors at one site, or multiple sites and supervisors at once. Your hours will automatically populate on separate pages for each site and supervisor.
Submitting your application
Once you’ve met all your license requirements, you’re ready to submit your application to the BHEC.
You’ll need to submit the following along with your application for licensure:
- Completion certificate for the Texas Jurisprudence Exam for Professional Counselors
- Supervised Experience Documentation Form
- Fingerprinting
- National Practitioner Data Bank (NPDB) self-query report
- Official transcripts
When you have all the required documentation, you can navigate to the Online Licensing System to apply.

How can my supervisor(s) sign my form?
At this time, your supervisor(s) won’t be able to electronically sign forms in Track Your Hours. You’ll need to download and send forms to your supervisor(s) to approve and sign outside of Track Your Hours.
Some customers choose to have their supervisors sign forms by hand. Others may email copies of the form and have their supervisors print, sign, scan, and email them back.
Which forms should I submit with my application?
The BHEC requires that you submit signed copies of the Supervised Experience Documentation Form with your license application. You must submit one form for each supervisor.
Completion of the Supervision Log is optional. You can submit this with your application.
The Notes report is created by Track Your Hours and shouldn’t be submitted with your application.
What if my submitted application is missing documentation?
According to the Licensed Professional Counselor Act, applications with missing documentation are considered incomplete and will be held open for 1 year from the application receipt date. If you don’t submit the missing documentation within the year, your application will expire, and you’ll need to submit a new application and fee.
Do I need to keep receiving supervision after completing my experience hours and submitting my application?
Yes, the BHEC requires that LPC Associates continue to receive supervision until their LPC license has been issued. For more information, see Texas Administrative Code - LPC Associate License.
What can I do if I’m experiencing issues downloading a form?
